New State Forms

We are always looking for ways to improve our reporting process. I am pleased to announce that we have created a new online form to expedite Knight of the Month submissions.
When you use this new form, your information will automatically be forwarded to State Community Director Larry Herman. Please use this form for all future submissions.
The new Knight of the Month and Year Report Form 26.09 can be found on the Michigan Knights of Columbus website under Resources → Community Support Materials.
New Supreme Family of the Month Form
All Family of the Month (FOM) entries should now be submitted using the new Family of the Month Form.
Please use this new form for all Family of the Month entries going forward. Due to ongoing issues, Do not use the Fraternal Program Report Form #10784 for Family of the Month submissions.
Please remember to continue sending your Family of the Month submissions to State Family Director Bill Corey at w.corey@mikofc.org.
